Fillable format for the forms

Forms are only printable right now, cant fill anything out. Want to be able to fill them out from the computer.

This would be very helpful for the PREVIOUS EMPLOYEE SAFETY PERFORMANCE HISTORY form. The form is not in a friendly format to be completed in writing and sent to previous employers.
